To create an action plan, please follow the steps below:

  • Log into the reporting site.

  • Click View Action Plan on the left-hand side of the page.

  • Click on the Create Action Plan button.

  • You'll notice a Pencil icon appears next to each question. Click the Pencil for the question you'd like to focus on.

  • Fill in the blanks as you see fit and click save.

  • To go back into that action plan you just entered, click on the (now highlighted) pencil icon, edit as needed, and click save.
