This will prompt you to enter your Username or Email address. Within 1-2 minutes, you should receive an email with a temporary password (we must have a username or email on file with Perceptyx in order for this function to work properly).

- Name of your Employer
- Your full name
- Your email address
- Your Employee ID or Username
