To create an action plan, please follow the steps below:
Log into the reporting site.
Click View Action Plan on the left-hand side of the page.
Click on the Create Action Plan button.
You'll notice a Pencil icon appears next to each question. Click the Pencil for the question you'd like to focus on.
Fill in the blanks as you see fit and click save.
There is not an actual "Submit" button or a way to show you've "completed" the Action Plan.
To go back into that Action Plan you just entered, click on the (now highlighted) pencil icon, edit as needed, and click Save.
